This form serves three purposes:

It gives us your information all in the same place, so we don't have to go back and forth asking and answering standard questions.

It helps us ensure that you get a companion that is appropriate for you.

It makes sure the bird goes to a safe, educated home.
Note that filling out this form does not obligate you to buy a bird, nor does it obligate us to provide you with one. We believe many different types of households and methods of husbandry are completely acceptable. However, we will not place an animal with someone who is set on unsafe/unhealthy practices or who is not prepared for the bird they are asking for. We will also not cater to anyone who feels the need to bully us about getting a bird, our prices, or anything else.
